Efficient Care Matching for CLB
Context
The CLB is a network of pupil guidance centres that supports students and their families with various challenges throughout their school career. One of the services provided by the CLBs is referring clients to external care institutions to help address specific issues.
Each CLB maintained its own lists and registers of service providers. This resulted in fragmented information, duplicated work, and outdated data. In addition, it was difficult to quickly find the right organization and to share this information with clients in an efficient and secure way.

What did we achieve?
We developed a custom web application with a central database that can be managed by all CLB branches. As a result, there is now a single shared register of care institutions, allowing updates to become immediately available to all involved parties.
This platform was initially developed for the independent CLBs in Flemish Brabant and Brussels, but was soon expanded to include CLBs from the other educational networks. More recently, the CLBs in Limburg also joined the platform. To support this, we created a structure in which the register is maintained centrally while the responsibilities for data maintenance are distributed. At the same time, the platform remains personal: every advisory document generated for clients uses the template of the appropriate organization. In this way, the workload is shared while the output remains personalized and tailored to the different networks.
During consultations with clients, staff members can quickly find suitable service providers in the system based on a broad range of parameters. They can then automatically and securely send this information with a single click in a clear and well-structured PDF document. To ensure data quality, a data refresh process was implemented in which service providers are involved in keeping their own information up to date. In addition, all modifications are submitted for validation to the moderators responsible for the relevant area of expertise.
